Antithrombin III (ATIII), also referred to as antithrombin, AT, factor Xa inhibitor, or heparin cofactor, is a single-chain glycoprotein found in normal serum at a concentration of about 10–20 mg/100 ml. Its levels are higher in plasma than in serum due to its interaction with thrombin during the coagulation process. ATIII acts as a key regulator by inhibiting proteolytic enzymes involved in blood coagulation and fibrinolysis, including factor Xa, plasmin, thrombin, and trypsin. Its inhibitory function is significantly amplified in the presence of heparin. Clinically, decreased levels of ATIII are associated with a state of hyper-coagulability, increasing the risk of excessive blood clotting.
